I've been using this website for some time. It's absolutely simple to use and provides one with the choice of using PDF or Microsoft "Word".
There are many such websites but also many of them screw things up .... oddly ThaiVisa is one of those which are nothing by annoying.
(I especially like this Form: "Application for Visa on Arrival for Chinese Nationals (TM.88 Chinese)")
