Several friends say good things about Chiang Mai Dental Hospital (http://www.chiangmaidentalhospital.com/) It's at the front of Siamrad hospital on the superhighway, near Khwangsing intersection (Chang Puek/Chottana Road).

Also popular is Grace (http://www.gracedentalclinic.com/) in Nimman area. For those on a budget I know some Thais who use Dr Worrapojn Clini, 74/1 Chiang Saen Rd (Near Chiang Mai Gate)
