I have no clue from/to Samui and Pattaya.
Bangkok Airways, I believe, is the only airline that flies any direct flights between Chiangmai and Samui. You could check it's website for days/times/etc.

I think you will find that only Bangkok Air and Thai Air fly to Ko Samui.
I have used this agency for Thailand domestic flights before - http://www.traveller2000.com
Bangkok Air has flights from Samui to Utapao (near Pattaya), they tend to be cheaper than the Samui - Bangkok flights. I think it's about 2400 Baht each way.
You might as well book directly with www.bangkokair.com (http://www.bangkokair.com) as agents will charge roughly the same price.
I've not flown with them before, but my bf has been several times on company expenses and raves about the in-flight coffee : ) .
For Samui - Chiang Mai you can go with Bangkok Air (about 6000 Baht One Way) or Thai Air (about 5000 Baht). You could alternatively get a taxi to Surat Thani airport then fly to Bangkok, then Chiang Mai with Air Asia. That'd only cost you about 3000 Baht but it takes a few more hours.

WikiTravel - www.wikitravel.org (http://www.wikitravel.org) - will usually tell you, which airlines fly in and out of any place. I usually find out, which airlines fly the routes, then go to the airlines' own websites to check times/fares. I have never used a travel agent before... funnily enough.

I think Bangkok Air has a lock on flights to Samui and the Samui/U-Tapao route.
Utapao is about 30-40 minutes from Pattaya.
Bangkok Air also has an exclusive to the airport near Sukhothai.
I find them pricey and would only use them (and have used them) for these 2 destinations.
EDIT: Oh, Bangkok Air also has a Samui-Chiang Mai flight...

Bangkok AIrways does run special from time to time. I paid something like Bt850++ for a one way ticket from Sukhothai to BKK for November travel.

No. TG has also flown there for a long time already.

OK, I stand corrected.
I was there last in 2006..... :dontknow:
But I think only Bangkok Air does the Samui -Utapao run and the Samui-Chiang Mai.
From quickly perusing their website, Thai Air seems only to do Samui-Bangkok.
